Finding the right path to recovery

Whatever challenges you may be facing, we want to help you find the right programs to fit your unique needs. Intake and Engagement provides initial screenings for new clients or for those individuals who may be re-engaging with our services.

We use a number of assessments depending on your needs. Our assessment tools include dual mental health and substance abuse assessments, the Child and Adolescent Needs and Strengths (CANS) assessment and the Safe Schools assessment. 

There are a number of ways to access and enroll in our programs:

  • Prineville office: Walk-in or call to schedule an appointment (541) 323-5330
  • School-based Health Center: Any youth enrolled in school can walk into the Crooked River Elementary School at 757 E 1st Street in Prineville to access mental and behavioral health services through LCS Northwest. Medical services are provided by Mosaic.
  • Bend- Mental Health office: Call to schedule an appointment (541) 323-5332
