Crook County Intellectual / Developmental Disabilities Services

At the Crook County CDDP, we authorize and coordinate home and community-based services for individuals with intellectual and developmental disabilities. Our services help people live healthier and more independent lives in their homes and communities.We provide case management services for more than 60 people throughout Crook County.
To receive services through the CDDP, individuals must meet eligibility requirements for Intellectual and Developmental Disabilities services funded by the State of Oregon, Office of Developmental Disabilities Services (ODDS). We assist individuals and/or families to complete the eligibility application process, and if determined eligible, provide the individual with person-centered planning, needs assessment, and coordination of available services.
Crook County CDDP also coordinates Adult Protective Services and licensing of foster homes for individuals with intellectual and developmental disabilities.
